Hill Country Transitional Care Medicine is a physician-founded and locally owned practice dedicated to redefining how patients transition from hospital or post-acute care back home. Built on decades of experience in hospital medicine, critical care, hospice, and community health, our model blends compassionate clinical leadership with innovative remote patient monitoring and timely follow-up to keep patients safe, supported, and thriving where they heal best—at home.

We focus primarily on the vulnerable period after hospitalization, providing a reliable bridge between inpatient care and primary care. Under the direct supervision of our collaborative care model, patients receive close monitoring, rapid intervention when concerns arise, and seamless care coordination that reduces preventable readmissions and promotes long-term stability.

Our work strengthens the entire continuum of care. Primary care teams and specialists gain real-time insights and additional support for managing complex patients—without increasing office visits or wait times. Hospitals and post-acute partners benefit from improved continuity and safer discharges. Our Founders

Anthony Basel, DO

Co-Founder and Chief Executive Officer

Dr. Anthony Basel is the Co-Founder and CEO of Hill Country Transitional Care Medicine with dual board certification in Internal Medicine and Critical Care, bringing a unique blend of critical care and hospitalist expertise to the heart of the Hill Country community. As a founder of Hill Country Transitional Care Medicine, Dr. Basel is committed to bridging the gap between hospital discharge and outpatient follow-up—ensuring that recently hospitalized patients receive timely, coordinated care even when they don’t yet have access to a primary care provider.

Having served over a decade in the U.S. Army—including combat deployments and leadership of intensive care units—Dr. Basel understands the complexities patients face after serious illness or injury. Now serving as ICU Director for Christus Santa Rosa New Braunfels and San Marcos, he applies that same mission-driven approach to transitional care, helping prevent readmissions, reduce complications, and improve long-term outcomes for patients in our community.

Dr. Basel’s approach is rooted in accessibility, responsiveness, and collaboration. He works closely with hospitals, case managers, and post-acute teams to identify high-risk patients and provide them with the support they need—whether through in-person visits, remote monitoring, or telemedicine follow-up. His deep experience managing multi-organ failure, complex co-morbidities, and post-ICU recovery uniquely positions him to care for some of the region’s most vulnerable patients during a critical phase in their healing journey.

Erin Wright, MD, FAAFP

Co-Founder and Chief Operating Officer

Dr. Erin Wright is the Co-Founder and COO of Hill Country Transitional Care Medicine, bringing nearly two decades of clinical leadership in hospital medicine, hospice care, and community health to a new model of patient-centered transitional care. Her work focuses on creating safer, smoother recoveries for patients moving from hospital or post-acute settings back home through physician-supervised monitoring, timely clinical intervention, and close continuity of care.

As a board-certified Family Medicine physician and experienced health-system leader, Dr. Wright has directed multi-site hospitalist programs across South Texas, strengthening care quality, improving communication across teams, and developing systems that reduce preventable readmissions. Her leadership roles include System Medical Director for CHRISTUS Schumacher Partners and Hospital Medicine Director at CHRISTUS Santa Rosa – New Braunfels, where she has been widely recognized for advancing operational efficiency, clinical documentation, and collaborative care.

Dr. Wright’s commitment to compassionate, community-rooted medicine shapes the foundation of Hill Country Transitional Care Medicine. She has served as Medical Director for Hope Hospice since 2018 and as Medical Director for Volunteers in Medicine since 2023. She is a respected physician leader in various organizational roles and has been honored with multiple awards for compassion, courage, and humanitarian service. These experiences shape her belief that transitional care must be accessible, proactive, and deeply connected to the patient’s goals and home environment.
